Adastra Health Care – Orion Center is an acute physical rehabilitation facility nestled within a residential community that provides health care services for people of 18 years and older with life-altering injuries and medically complex illnesses. Our individual care plans are managed by an interdisciplinary group of physicians, nurses, physical therapists, occupational therapists and speech therapists, social workers, activity directors, respiratory therapists and registered dietitians.

Continuous (24-hour)
skilled nursing and supportive care provided by licensed healthcare professionals under the supervision and direction of a board certified Medical Director (MD/DO).
Subacute Rehabilitation
for physical, occupational and recreational therapy provided by licensed physical therapists.
Frequent / Intermittent
vital signs, neurologic and/or vascular monitoring requiring 24-hour care.
Ventilator
or tracheostomy dependence/complex pulmonary care that cannot be provided at a lower level of care.
Intermittent Respiratory Therapy
services for oxygen delivery, suctioning, CPAP or BiPAP.
Dietary Care
including tube feeding or total parenteral nutrition (TPN) with other medical co-morbidities not manageable at a lower level of care.

Our 24-hour skilled nursing and case management services are provided with multilingual Licensed Vocational Nurses who speak English, Spanish, Armenian, Russian and additional languages. Staff ratio is 1:3.
Our newly remodeled facility has a fully equipped kitchen, entertainment area, rehabilitation room for therapeutic activities, a fully landscaped backyard and spacious rooms. Rooms are tastefully decorated and have hardwood flooring and closets. Each room has its own entry and exit door that leads to fully landscaped grounds, so that individuals and their families can spend quality time together.
Our mission is to achieve an environment where our resident and their family see themselves not as patients but rather as members of our extended family. We strive to meet all their needs on a 24-hour, 7-day basis.
